Please bring your ID/Driver's License and any relevant medical records related to your condition, especially if you have visited any other medical provider(s) prior to your visit. If you plan to use health insurance, please bring your insurance card with you or call us at (510) 220-8211 so we can check your plan's coverage and benefit.
What to wear
Please wear comfortable, loose-fitting clothing around the areas where you are currently experiencing pain.

You can either fill out the new patient intake forms online or in person prior to your first visit. Completing your paperwork online will help to streamline your experience during your first visit so we can treat you sooner.
